Staying Well Plans

Staying Well Plans can help people to stay well at work, taking into account individual needs and preferences and supporting managers and staff to have meaningful conversations together around health and wellbeing.

There are two versions of the plan:

Staying Well Plan - Full Version Template. This can be used:

  • as part of the induction process
  • with staff who have a long-term health condition
  • with staff who develop a health condition and need to have adjustments made

Staying Well Plan - Brief Version Template. This can be completed as part of the annual “Your Conversation” appraisal process.
