This review brings together the learning from Population Health Management projects delivered across the programme to date. It provides a summary of the work undertaken, the outcomes achieved, and the insights gained through applying PHM methodologies to understand population need, target interventions, and reduce health inequalities.

The review identifies key themes and lessons emerging from the projects and outlines strategic recommendations to inform future priorities, support system-wide decision-making, and embed a sustainable, evidence-based approach to improving population health and wellbeing.
