CTM Memory Service/Dementia Programme The Memory Service offers a comprehensive assessment for patients suspected of having cognitive difficulties due to a progressive condition. Following a diagnosis, individuals and their care partners will be supported with guidance in coming to terms with the diagnosis, functional and practical advice on living well with dementia, and signposted to third sector services.
