As a Health Board, we understand that the period following the death of a loved one in hospital is a deeply sad time for family and friends. We aim to support you through the difficult time ahead and give you the information you need to register a death. There are some things you need to do to prepare for this.
When you feel ready, you will need to contact our Bereavement Team at the relevant hospital:
The team will explain what happens next. The team will ask you for some details about the person who has died. They will ask for your details, your relationship to the deceased and confirm you are the person arranging the funeral. They will also ask you if you have a chosen funeral director.
The team is available Monday to Friday between 9am and 4pm. The teams in our community hospitals (YCR and YCC) are available between 9am and 12.30pm. The Bereavement Office is not available over the weekend or on Bank Holidays. If you are unable to speak to someone, please leave a message and the team will get back to you.
Issuing the Death Certificate
Every death requires scrutiny before a death certificate can be issued; this is carried out, either by the Medical Examiners Service, or the Coroners Service depending on the circumstances
There are currently some delays in this process and this may mean that receiving a death certificate may take slightly longer than usual. These delays are outside of the control of the Health Board, but our Bereavement Team will be able to advise you of current timescales when you contact them.
