Information for referrers

At present, we currently have slightly different referral pathways for Merthyr/RCT and Bridgend, although in connection with service users and other agencies, we are looking to unify and relaunch a new referral pathway over the course of 2024. Please see below for our current process:

In Merthyr/RCT

We prefer that referrals come from the professionals who know the child well. For pre-school children, this will usually be a joint referral from the family and the child’s Health Visitor. For school-aged children, this will usually be a joint referral from the family and the child’s school. For referrals made by other professionals, we will always require information from the education setting to support the referral information.

For children and young people who are not attending school, we will consider referrals made jointly by the family and another health/education/social care professional.

In Bridgend

Referrals for pre-school children are made through our multi-disciplinary PAD process. Please discuss this further with the child’s Health Visitor.

Referrals for school-aged children are made through our schools ND pathway. Please discuss this further with the child’s school. We are currently working on a pathway for school-aged children who are not attending school.
