This year's Future Care Planning Week theme is One Meaningful Conversation.
Future care planning is about thinking ahead and talking about the kind of care and support you would want in the future, especially if you became unwell and couldn’t communicate your wishes.
These conversations do not need to be formal or frightening. Often, they begin with a simple question, a cup of tea, and someone taking the time to listen.
At Cwm Taf Morgannwg, we are committed to delivering care that is centred around what matters most to people. Future Care Planning supports this by helping individuals share their wishes, values and preferences, ensuring their voice remains at the heart of decisions about their care.
Throughout the week, our Future Care Planning Specialist Nurses will be visiting hospitals, community venues, libraries, care homes and healthcare settings across Cwm Taf Morgannwg to encourage conversations with staff, patients, families and communities.
