Older Person's Assessment - Treatment Wards
What We Do
Provide assessment and treatment to adults with a diagnosis of dementia, or older adults aged 65+ suffering with all mental health illnesses as part of a multi-disciplinary team approach looking at all areas of daily living.
St David’s and Seren units are based at the Mental Health Unit at the Royal Glamorgan Hospital. Both wards cover four locality areas of Cwm Taf Morgannwg UHB and include Rhondda, Taf Ely, Cynon and Merthyr.
Can anyone use this service?
Older person’s wards are primarily aimed at adults aged 65 and over with a mental illness, or any age with a confirmed diagnosis of dementia.
Admission to these wards is via the Consultant Psychiatrist for each locality, or Senior Management Team.
Out of hours admission can also be made via Crisis Teams Rhondda and Taf Ely Team and Merthyr and Cynon and the Mental Health Liaison Teams based in RGH & PCH.
If open to other services, a copy of all relevant information should be provided to the ward which would include an up to date risk assessment and Care and Treatment Plan and will remain involved throughout the admission to ensure continuity of care throughout.
What to expect
We provide a multi-disciplinary approach assessment and treatment plan to enable the patient to achieve optimal level of functioning/ recovery. The multi-disciplinary team involved in this process will include nursing staff, Consultant Psychiatrist, Psychology, Occupational Therapy, as well as our local authority colleagues within social care, advocacy and community mental health teams.
We provide information and advice and will refer to third sector services where beneficial.
A named nurse will be allocated to co-ordinate your care and maintain a contact point for families.
Ongoing assessments may include community based assessments such as home visits or local community visits.
