Mental Health Liaison Team - Merthyr and Cynon

What we do

The Mental health Liaison Team (MHLT):

  • Provides specialist mental health education and support to staff within the inpatient setting and Emergency Department (ED).
  • Provides mental health support to adult individuals admitted to hospital with medical conditions/planned operations that have co-morbid psychiatric conditions.  Example referrals from services may be for individuals with chronic/acute medical conditions complicated by behavioural or psychological disturbance, depression, dementia, anxiety disorder, psychosis and for those who present with self-harm.
  • Supports individuals presenting with alcohol and/or substance misuse and aide in the management of withdrawal and signposting to specialist substance misuse services.
  • Following assessment the MHLT will engage and signpost to other relevant services and agencies in order to provide forwarding appropriate support and to facilitate safe discharge.

 

Can anyone use this service?

The service is for hospital inpatients of adult age with presenting medical and mental health needs from Prince Charles Hospital and Ysbyty Cwm Cynon

What to expect

  • Professional response with appropriate requested referral time. 
  • Every referral from ED will aim to be seen within one hour.
  • Complete WARRN risk assessments & risk management plans for all patients assessed.
  • Early detection and support for patients with mental health decline to facilitate rapid and appropriate intervention including use of the mental health act.
  • Continuity of care for people already known to mental health services.
  • Advice on medication/treatments.
  • Help with discharge planning, general advice and support.
  • Signposting to support sectors including psychiatric inpatient care.
