Older Person Mental Health Liaison and Care Home Outreach Team (Bridgend)

What we do

The Older Person Mental Health Liaison Team (OPMHLT) and Care Home Outreach Team:

• Provides mental health support to older adult individuals admitted to hospital with medical conditions/planned operations that have co-morbid psychiatric conditions and the case management of patients within the Bridgend area in care and residential home who require assessment. Example referrals from services may be for individuals with chronic/acute medical conditions complicated by behavioural or psychological disturbance, depression, dementia, anxiety disorder, psychosis and for those who present with self-harm.
• Supports older adults presenting with alcohol and/or substance misuse and aide in the management of withdrawal and signposting to specialist substance misuse services.
• Following assessment, the OPMHLT will engage and signpost to other relevant services and agencies in order to provide forwarding appropriate support and to facilitate safe discharge.
• Provides specialist mental health education and support to staff within the inpatient setting, care and residential home and Emergency Department (ED).

Can anyone use this service?

The service is for older adult hospital inpatients with presenting medical and mental health needs from Princess of Wales Hospital and Bridgend Outreach for care homes and residential homes in the Bridgend area.

What to expect

• Professional response with appropriate requested referral time. 
• Every referral from ED will aim to be seen within one hour.
• Complete WARRN risk assessments & risk management plans for appropriate patients assessed.
• Early detection and support for patients with mental health decline to facilitate rapid and appropriate intervention including use of the mental health act.
• Continuity of care for people already known to mental health services.
• Advice on medication/treatments.
• Help with discharge planning, general advice and support.
• Signposting to support sectors including psychiatric inpatient care.
