Information for referrers

Following a period of consultation and collaboration with parents/carers and Education staff, we launched our new referral paperwork for school-aged children in January 2025.  We now have the same paperwork and processes across Merthyr, Rhondda Cynon Taf and Bridgend.

For children and young people attending school, we will need our paperwork completed by the family and staff in school to consider a referral:

Forms can either be completed as Word documents and emailed to us, or via our Microsoft Forms option (embedded in the forms above).

For children and young people who are not attending school, we will consider referrals made jointly by the family and another health/education/social care professional.  Please contact us to discuss this further.

For pre-school children, we suggest that referrals come from the child’s Health Visitor or Early Years Educational Psychologist, in collaboration with the family.

From February 2026, we introduced a new referral form that Health Visitors can use in conjunction with families to request some input from our Community Paediatricians/Neurodevelopmental Team.
