Autumn booster – Think you have been missed?


We have sent all of our appointments to everyone who is eligible for an autumn booster. However, do you think we have missed you?

We remain committed to leaving nobody behind, so if you feel you belong to one of the eligible groups and are yet to receive an appointment for us, please fill in the form below.

A couple of things before you do:

To process an appointment for you, we will need to know which group you believe you are in.

A member of the team will either contact you (email or phone) or send you a new appointment. Please allow three days for the team to action your request and seven days for your appointment letter to arrive.

There should be an interval of at least 13 weeks between having COVID-19 vaccines. Please do not worry if you have had a vaccine within 13 weeks, our team can still make you an appointment, but will make sure at least 13 weeks has passed.
