Autumn booster – Carers appointment form

If you are carer and aged 16 and over you are eligible for a COVID-19 autumn booster.

We have now sent all our appointments for the autumn booster (November 2022). If you have not had an appointment letter from us, then please get in touch by completing the form below. We may not have known that you are an unpaid carer.

A member of the team will either contact you (email or phone) or send you a new appointment. Please allow three days for the team to action your request and seven days for your appointment letter to arrive.

There should be an interval of at least 13 weeks between having COVID-19 vaccines. Please do not worry if you have had a vaccine within 13 weeks, our team can still make you an appointment, but will make sure at least 13 weeks has passed.
