We formed as a virtual network In December 2021, before moving to a community-based meeting in February 2022.
This regional partnership forum of 130 members, connects third sector partners and community organizations across Cwm Taf Morgannwg to our Health Board’s CTM2030 ambition of improving population health outcomes through a ‘Building Healthier Communities Together’ approach.
The Network is key to engaging our communities and third sector partners in the population health agenda; looking at how we deliver the best care and services possible for our patients and communities, how we ensure communities feel fully informed of the support and services available to them, and how we can work in partnership to support people to manage their own health and care, as independently as possible.
Our meetings are hosted by our Chair, Jonathan Morgan and CEO, Paul Mears, and supported by other senior leaders.
We meet in the heart of Cwm Taf Morgannwg every 3 months, with meetings rotating across Rhondda Cynon Taf, Bridgend and Merthyr Tydfil.
