Older Persons Community Mental Health Team
What We Do
The older adult community mental health team endeavours to deliver evidence-based holistic care to individuals within the context of a multi-disciplinary, multi-agency community mental health service.
We provide comprehensive assessment, care and treatment to individuals who are living with complex mental health needs in accordance with Part 2 of the Mental Health (Wales) Measure 2010.
We promote a positive working culture, working collaboratively with service users, ensuring that their views and ideas are heard and equal in developing a range of user led initiations to support the individual and their carers.
We foster robust, open and positive working relationships with all partners in particular our Local Authorities and other mental health services and the third sector, ensuring that all partners are treated with dignity and respect.
Can anyone use this service?
Secondary care community mental health services can be referred to by GPs, Local Authority partners and other professionals for consideration.
Self-referrals cannot be made directly.
What to expect
We provide care and support to individuals with complex functional mental health needs aged over 65 years, or those of any age with a diagnosis of dementia.
We are a multi-disciplinary team and individuals will have access and assessment provided by psychology, medics, mental health nurses and occupational therapists as required.
Once a robust assessment has been completed, each individual will have a care plan which will identify what care and treatment is required and who will be responsible for the delivery of care. An annual review will be completed by the named care coordinator prior to discharge from the service.
The service will respect individuals’ rights to exercise control over their own lives, care and ability to make choices in an informed and safe way, whilst aiming to promote the values of independence, choice and empowerment for service users.
