Awen Hub will provide support for individuals in the Rhondda Cynon Taf and Merthyr Tydfil area, who
have persistent neurological problems, either as a result of a health-related condition or a new acquired injury to the head.
OUR AIM
The service will focus on supporting individuals to optimise their psychological and cognitive
functioning.
MILD TO MODERATE NEUROLOGICAL DIFFICULTIES
Awen Hub defines mild to moderate neurological symptoms or conditions, as disability which does not
require multidisciplinary input to support rehabilitation goals. This definition allows for us to be needs
focused, trans-diagnostic and to focus on promoting self-efficacy and autonomy.
REFERRAL CRITERIA
We will accept:
- A mild to moderate acquired neurological difficulty as assessed by a medical doctor.
- Non-progressive or relatively stable neurological conditions.
- Psychological or cognitive issues associated with the neurological condition.
- Must be willing to work on specific, time limited goals to promote autonomy.
- Individuals must be able to attend clinic, groups and/or community activities.
- Must be residents of Rhondda Cynon Taf or Merthyr Tydfil areas living in the community.
- Must be aged 18+ and consent to the referral.
We are unable to accept:
- Moderate to severe brain injury or neurological disability requiring multidisciplinary support.
- Conditions where progressive functional decline is anticipated with increasing multidisciplinary. support and care needs.
- A spinal cord or peripheral nerve condition only.
- Congenital/developmental conditions.
- Physiotherapy and SLT needs only.
- OT functional rehabilitation, upper limb work, dexterity, for home adaption or basic ADLS.
- Mental health needs requiring multidisciplinary input or risk management.
- Substance use as a barrier to engagement.
- Primary support needs that are unrelated to a neurological condition.
- Receiving a service from other therapeutic neurological services.
- Functional neurological and TIC disorders.
- Specific return to Work/education focused support and advocacy.
- Support with PIP and/or benefit claims.

INTERVENTIONS
The service will offer a range of interventions:
- Psychological assessment, formulation and time limited therapy
- Cognitive assessments
- Virtual Psychoeducational workshops
Therapy sessions will be offered from Keir Hardie Health Park in Merthyr Tydfil and Ysbyty Cwm
Rhondda.
DISCHARGE
Discharge will occur when:
- When goals have been reached
- When the individual no longer requires support, does not engage or opt into the service
We welcome re-referral for discharged individuals who identify a new rehabilitation goal
SERVICE OPENING HOURS
Our opening Hours are Monday – Friday (9am – 5pm).
Outside of these hours we operate an answer machine and email service which will be picked up the
next working day.
If urgent medical attention is required or if a person is experiencing crisis. Please contact their
registered GP or the Out of Hours GP Service.
