Glyncornel Nursing Home in Tonypandy have successfully completed the Six Steps to Success education programme supporting Palliative and End of Life Care supported by the Specialist Palliative Care Team in CTMUHB who piloted the programme as part of the Bevan Commissions Adopt and Spread Programme.
Focusing on care homes and home based care teams to achieve quality improvement outcomes and quality experiences for individuals and families at end of life, the Six Steps to Success program aims to increase knowledge, skills, confidence and access to evidence based palliative and end of life care throughout the education programme. The program using six core standardised workshops, advance care planning, anticipatory care planning, quality end of life care and bereavement support within their care settings.
