Glyncornel Nursing Home celebrates success with the Specialist Palliative Care Team in CTMUHB

Glyncornel Nursing Home in Tonypandy have successfully completed the Six Steps to Success education programme supporting Palliative and End of Life Care supported by the Specialist Palliative Care Team in CTMUHB who piloted the programme as part of the Bevan Commissions Adopt and Spread Programme.  

Focusing on care homes and home based care teams to achieve quality improvement outcomes and quality experiences for individuals and families at end of life, the Six Steps to Success program aims to increase knowledge, skills, confidence and access to evidence based palliative and end of life care throughout the education programme. The program using six core standardised workshops, advance care planning, anticipatory care planning, quality end of life care and bereavement support within their care settings. 

Lisa Wooler, Macmillan End of Life Care Modernisation Manager said “Jacqui and Serena and all the staff in Glyncornel Nursing Home should be really proud of this achievement, despite the many obstacles that the pandemic presented in delivering the programme and those faced by the care homes themselves they showed commitment to support their residents to achieve what matters most to them at the end of life.” 

Rebecca Spicer-Thomas, Senior Nurse for Specialist Palliative Care & A/FCP added “It is evident the training has proven helpful. Jacqui and Serena have shared what they learnt with their team and empowered the staff and residents in the home to openly talk about their wishes plans for end of life; all the residents now been offered the opportunity to share what matters most.”
