Who does CTM UHB Value Based Health Care support?

The CTMUHB VBHC Team currently provides funding and management for project and workstreams across the Health Board, including the following areas:

  • Heart Failure
  • Lymphoedema
  • Diabetes
  • Spirometry
  • Alcohol Liaison Service
  • AF and Hypertension

The Team also provide guidance and sign-posting on VBHC concepts and principles, including the use of VBHC measures (PROMs, PREMS, etc)
