Patients who are supported by the new Wellness Improvement Service (WISE) at Cwm Taf Morgannwg health board, will from today have personal access to a tailored digital social prescribing platform.
A new partnership with Elemental Software, an award-winning social prescribing technology company, will help assist patients with chronic health conditions to better connect into community-based programmes, services, and healthcare interventions.
With digital tools now extensively being used to supplement face-to-face healthcare, the adoption of Elemental’s digital platform will enable WISE to help support patients access several opportunities to improve their physical and mental health outcomes, promote wellbeing, and support the reduction of health inequalities across the Health Board’s regional population.
WISE forms part of the NHS Wales Planned Care Programme for Cwm Taf Morgannwg Health Board and follows an evidence-based, lifestyle medicine approach. Patient empowerment forms the basis of the service and supports behaviour change through person centred techniques to improve mental and physical wellbeing.
Through a sustained education and coaching programme, WISE enables referred patients to better understand the root causes of their current medical conditions and choose lifestyle behaviours that improve their long-term health, as well as ensure a better quality of life with reduced symptom burden.
