Care Closer to Home

What is the Care Closer to Home (CCTH) Service?

This service provides care out of hospital and in your home for your child. The CCTH service is run by community children’s nurses (CCN).

Services provided include:

Post-surgery wound checks
Wound care following injury
Monitoring /redressing of burns
Replacement of naso gastric tubes
Prescribed Injected medication administration
Phlebotomy Services
Surveillance for certain medical conditions e.g HSP

Referrals

Referrals can be accepted from a medical/nursing/therapy professional. Referrals should be sent to:
