Earlier this week, the Cwm Taf Morgannwg UHB Quality and Patient Safety team held a Listening and Learning event for CTM staff members.
The purpose of the event, which is the first in a series, was to explore with staff from across the Health Board how we can better learn from incidents and events to improve patient outcomes.
During the event, CTM’s new Listening and Learning Framework was launched. The Framework brings structure to the way incidents and the learning from those events is captured, ensuring consistency across the Health Board.
Louise Mann, Assistant Director for Quality and Safety said: “ It is important that there are robust and reliable processes in place to effectively manage adverse events, and demonstrates that we listen to our patients and workforce to ensure that lessons are shared widely and used to support improvements in care and service delivery. We want to achieve a positive safety culture that is open, just and informed, in which reporting and learning from error and successful practice is the norm.
“These events along with our new Framework ensure that each member of our teams has access to the information and learning than they need to continually improve the services we provide.”
