Access to Health Records Requests

If you wish to access your own health record, a record on behalf of someone else or a person who is deceased, you will need to contact the Medical Records Department. The email address for this department is CTT_Medrecordrequest@wales.nhs.uk.

Individual Patient Funding Requests (IPFR)

Your GP or hospital consultant can ask us, on your behalf, to fund a treatment that we would not normally provide for you. This is called making an Individual Patient Funding Request (IPFR)

Making a Request for Treatment in the European Economic Area (EEA)

To seek prior authorisation for a treatment in the EEA or to apply for reimbursement, patients will need to complete the appropriate application form and submit the evidence required that demonstrates residency and clinical need.

When applying for reimbursement, original receipts and proof of payment directly to the EEA healthcare provider must be supplied.

Make a Donation

Cwm Taf Morgannwg University Health Board operates the Cwm Taf Morgannwg NHS General Charitable Fund [Charity number: 1049765] as a vehicle to receive charitable donations from the general public. Monies from this fund can be used for a variety of charitable purposes that cover patient and staff welfare, the purchase of equipment, and research and education.

Individuals who wish to make a donation may do so at any General Office for the hospital of their choice. Without imposing any trust, the purpose of any donation will be recorded on the receipt issued to the donor by the General Office. The monies, where practicable, will be used for the recorded purpose.

Cheques should be made payable to Cwm Taf Morgannwg University Health Board. Or you can also donate via our Just Giving page.

For Health and Safety and operational reasons, donations of equipment will not be accepted.

How to Make a Complaint

Your experience of our service is important to us. If at any time you feel dissatisfied with the treatment or care you have received, you have a right to raise a concern. In the first instance, if you feel able to do so, please raise your concern with a member of staff at the time and they will do their best to put it right immediately.
