Digital Health Assessments are clinical and experience assessments directly related to your care. You can complete these assessments online, on your phone, via paper or on a computer. Digital Health Assessments are forms that help us understand how you're feeling. They allow you to share information about your symptoms, physical health, and quality of life between appointments, enabling us to provide you with better care. You will receive the assessments at key times in your care pathway, such as before an appointment, after an appointment or operation, and while you may be on a waiting list.

Why is the Health Board collecting Digital Clinical Health Assessments and Experiences?

Sometimes it’s hard to remember everything about how you’ve been feeling over time. Regularly updating us from home helps us:

  • Get a clear picture of your health.
  • Ensure you have a bigger role in your healthcare.
  • Improve the treatments based on your feedback.
  • Understand how you're doing before and after appointments.
  • Involve you more in your own healthcare.
  • Learn more about how treatments are working for you and any side effects.
  • Make better decisions with you about your care.

When will I receive a Digital Health Assessment?

Here is what you can expect when it's time for your health assessment:

  • You will get an email or text with a link to the health assessment in both Welsh and English.
  • At 9:30 AM, you will get a call with a pre-recorded voice message telling you to complete your health assessment.
  • At 9:45 AM, you will get a welcome message with a link to the privacy notice and a link to the health assessment.
  • If you don't start the assessment within 24 hours, you'll get a reminder.
  • If you start but don't finish the assessment, you'll get another reminder.

How Digital Health Assessments work?
  • Notification: When your health assessment is due, we will send you a text and/or email message containing a digital link.
  • Access Link: Click the link in the message from "PROMPTLY".
  • Enter Mobile Number: Enter your mobile number and click "next".
  • Receive Code: You'll receive a verification code via text message.
  • Log In: Enter the code on your tablet or computer and click "log in".
  • Accept Terms: Accept the terms and conditions to see your available assessments.
  • Complete Assessment: Find the assessment you need to complete and click "Complete Assessment".
Can I view my results on the system?

Yes, you can see all your past responses and scores. Here is what you will see on the digital platform:

What devices can I use to complete my Digital Health Assessment?

You can view and complete your assessments on any device that has access to the internet. Devices commonly used include iPad, laptops, or mobile phones. Just click the link in the text message or email we send you and log in as usual.

Can I trust the text message and email link?

Yes, you can trust the link in your text messages and emails. They are sent by our partner, Promptly, who meets the highest NHS standards for protecting your health information.

I am struggling to log into my Digital Health Assessment?

If you can't log in, we might have incorrect information for you. Contact your care team or check the details on your appointment letter to make sure we have your:

  • Name
  • Date of Birth
  • Postcode
  • Mobile number
  • Email
Problems accessing your Digital Health Assessment?

If you see a red message saying there's a problem with your assessment, contact your clinical team or email ctm.vbhcoutcomes@wales.nhs.uk.

Who will review my answers to my Digital Health Assessment?

Your clinical team will review your answers. You and your clinical team can see how your answers change over time and track your progress.

How often will I need to fill in the form?

This depends on your health condition. Most digital health assessments are done before appointments and at agreed times after you leave the hospital. We will send you a text message or email when you have a new digital health assessment to complete.

What questions will I be asked?

The questions depend on your condition but may include:

  • Diagnosis and treatment
  • Pain management
  • Mobility
  • Symptoms
  • Quality of life
  • Daily living
  • Social activities
  • Well-being
  • Work status
  • Other medical conditions
What if there is a problem?

What should I do if I need medical care?

The form does not replace medical care. Seek medical advice immediately if you feel unwell or are concerned about your symptoms. Call 999 for life-threatening emergencies.

How is my data and information handled?

Your answers help your clinician understand your health and plan your treatment. Anonymous information may be used to improve services and support research. By submitting a Digital Assessment, you consent to this use. All data privacy rules are followed, for further information please click here.

Can I view the Digital Health Assessment in Welsh?

Yes, you can choose to view and answer the assessment in Welsh or English. There is a flag at the on the right-hand side at the top of the screen. This can be amended to your preferred language at any time. 

What if I accidentally decline the terms and conditions?

Contact your clinical team or email ctm.vbhcoutcomes@wales.nhs.uk to re-enrol.

Can I opt out of receiving text messages and emails for Digital Health Assessments?

Yes, you can opt out at any time. To do this click on the clog wheel at the top of the screen and click unsubscribe, as seen below:

Will this affect any payments I receive from the government?

No, digital health assessment scores do not affect government payments. They are used to understand your health and improve healthcare services. For concerns about payments, talk to your clinical team or a benefits advisor.

What if I need help to complete my Digital Health Assessment online?
