Innovative Bridgend care home project scoops award

A project which is cutting waiting times for care home patients in Bridgend who need access to multidisciplinary advice, has won an Advancing Healthcare Wales ‘New way of working’ award.

Four Allied Health Professionals from Bridgend Community Integrated Services, Cwm Taf Morgannwg UHB were successful in the awards which are an opportunity to recognise and celebrate the important and innovative work of healthcare scientists and allied health professionals across Wales.

This Bevan Exemplar project showcases a new integrated way of providing swallowing, nutrition and medication management to care home residents to improve clinical outcomes, reduce unwarranted hospital admission and improve job satisfaction for health and social care staff.

In this pilot phase, the service was made available to over 260 care home residents in the Bridgend area.  This is an integrated model that can be easily scaled up across Wales. 

The CTM team includes Sheiladen Aquino, Clinical Lead Speech and Language Therapist Thomas Sauter, Clinical Lead Pharmacist, Lucy Marland, Specialised Care Home Lead Dietitian and Amber McCollum, Specialist Speech and Language Therapist.

They said: “We are thrilled to have received the New Way of Working Award in recognition of the Integrated Nursing Home project in Bridgend. This will not only allow us to cascade the integrated way of working across care home settings in our health board area, but also start planning to scale up the project across NHS Wales.

“The project so far has helped save valuable time, money and resource and enhanced the service provided to care home residents in a collaborative patient and healthcare approach.”
