Community Childrens Nursing Team

What we do

The Community Children’s Nursing Team (CCNT) has bases in Keir Hardie University Health Park in Merthyr Tydfil, with a satellite base at Ysbyty Cwm Rhondda in Llwynypia and Glanrhyd Hospital Bridgend.
We provide a variety of generic nursing care and support for a number of patient groups, including those with acute health needs, chronic illness and in a Continuing Health Care capacity.
We aim to facilitate early discharge from hospital, prevent readmission and empower parents / carers, with support, to manage their child’s health needs.

Who is it for?

The team provides nursing care & support to children, young people and their families across the whole of Cwm Taf University Health Board, aged from birth to 16-years-old and up to 19-years-old for children with complex health needs.
Children and young people that use the service typically have a short or long term health need.  Some children with a short term need are supported via our Care Closer to Home service.

Can anyone use this service?

Referrals are accepted from medical, nursing or therapy professionals

What to expect

Following referral, the child and family will be visited at home by a registered nurse from the CCN team to undertake an assessment, determine what care is needed, plan and deliver that care until discharge or transfer of care is appropriate.
