Make an appointment for first, second, third and booster doses

During the autumn booster programme, we have paused our walk-in clinics for first, second, third or booster one doses.

That means you are unable to attend one of our community vaccination centres without an appointment.

However, it is really easy to request an appointment, please just check the information below to make sure you are eligible to receive one of the primary doses and then fill in the form.

We remain committed to making sure nobody is left behind for first, second, third or booster doses.

Please do not think it is too late – it is not.

Maybe you were unable to make your appointment for your first, second, third or booster – or you are now overdue your next dose – please come forward.

All of our community vaccination centres have available appointments for anyone aged 5+.

Before requesting an appointment, please make sure you are eligible (all the requirements are below) and that you have waited the right time between doses and accounted for the necessary time you need to wait after COVID-19 infection.

You can find which community vaccination centre is the nearest to where you live here:  Our community vaccination centres (CVCs) - Cwm Taf Morgannwg University Health Board (nhs.wales)
